When true, identifies the business with an EIN (Employer Identification Number).

Enter the Employer Identification Number (EIN) if Is EIN is true, or the Social Security Number (SSN) if Is EIN is false.

Enter the street address or post office box of the Employer/Payer

Enter suite or apartment number of the Employer/Payer.

Enter the city where the Employer/Payer is located.

Enter the 2-letter abbreviation of the state where the Employer/Payer is located.
