In xMatters, navigate to Developer > Manage Communication Plan and click 'Edit' on the Plan you want to use to create an Event.
Select the Form you will use and in the Deployment options menu, select 'Access Web Service URL'

The properties action field should map to the event properties you have added to your Communication Plan Form in xMatters.
In xMatters, navigate to Developer > Manage Communication Plan and click 'Edit' on the Plan you want to use to create an Event.
Select the Form you will use and in the Edit menu, select 'Layout' to see the Event Properties for that Form.
Priority
Setting the Event property in a Zap is optional.
If no Priority is defined in the Zap, xMatters will use the default priority of the Communication Plan Form.
