When you update a form, Facebook assigns a new form ID. You must reselect it from the dropdown menu.

Select the Facebook Page connected to your lead form. If you cannot see your form, find the [Facebook Page ID](https://www.facebook.com/help/1503421039731588) and set it as a [custom value](https://help.zapier.com/hc/en-us/articles/8496241696141-Add-custom-values-to-dropdown-menu-fields-in-Zaps).

Select the form associated with your Facebook Page. If you select “Default (Any Form)” you must grant [sufficient permissions](https://help.zapier.com/hc/en-us/articles/8496040397965-What-permissions-do-I-need-to-use-Facebook-Lead-Ads-with-Zapier) to all forms associated with the Facebook Page.

Use the 2-letter U.S. state code (e.g., CA, NY)

Location Name as a string. Ideally, it should be Location name like in the PM system

First name and Last name of Provider separated by space

Free form messages or notes coming along with the Lead data

Provide utm_campaign tag value if available.

Provide utm_source tag value if available

Provide utm_medium tag value if available

Provide utm_content tag value if available

Google click ID if lead is created from the Google Ads
